2.)  Who covers shipping?

If you utilize our account number, you pay inbound shipping and we pay return shipping with the exception of overnight rush cases.  Standard shipping is via UPS and we get great discounted rates.  We can receive UPS, FedEx, United States Postal Service, and SpeeDee packages.  UPS return shipping will be utilized, unless FedEx or SpeeDee is requested.  We do not send packages via United States Postal Service.  Below are our UPS rates:

3.)  Where do I get your shipping labels and work orders?

You can get our shipping labels and work orders right off our website.  Visit www.colonialdentalstudio.com/order and click on the “UPS Labels” to get the shipping label and click on “Work Order Form” to get our work order.  Once you fill out the required fields and select your shipping type, our UPS label will be sent directly to the e-mail address you enter in. 

We can also send these materials to your lab via mail, fax, or e-mail by request. 

 

4.)  What type of materials will be used with my cases?

 Materials can be found by visiting www.colonialdentalstudio.com.  Click on the “Products” tab to view the PDF file.  Zirconia certification stickers, Valplast certificates, iDentalloy stickers, etc. are all provided with your cases.

 

5.)  What is your warranty and remake policy?

For remake cases, most will come at no charge.  Below is a list of situations that would require additional charges.

1.)    If the original restoration is not returned.

2.)    If the case is prepped again, and a new impression is sent.

3.)    If the shade is different from that written on the original work order.

4.)    If the material is different from the original work order.

5.)    If a new impression is asked for by our technicians, and we are asked to proceed with the case without a new impression.

6.)    If we request a try-in and we are asked to proceed without one.

 

6.)  How does payment work between my lab and Colonial?

We will send an invoice back with each case.  On the last working day of each month, we process statements.  These statements list all of your invoices, payments, and credits from that month.  Statement balances must be paid before the last working day of the following month (the day the next month’s statements are processed).  If a balance remains outstanding, a 1.5% service charge will be added to that remaining balance and your account will be “30 days past due.” 

You may pay via check, money order, debit card, or credit card.  We accept VISA, MasterCard, Discover, and American Express.  We keep credit card information on account for all of our customers.  Our credit card and account activation documents can be found by visiting www.colonialdentalstudio.com/order.  If you foresee this being an issue, please call and we can discuss other options.
